• Select your empowerment status

• If you are benefits from any federal government Programme select YES is no select NO

• If yes enter the empowerment name

• If you convicted from any crime select YES if no Select NO

• If YES state the crime in 25 word

• If you can abide the low by P-Yes select YES if you can’t select NO

• If you are business owners select YES if you not owner select NO

• Enter what business your doing current select yes
